    VOLUNTEER clearances may NOT be used for employment at Fairfield Area School District.

    All new employees of the  Fairfield Area School District must obtain a Child Abuse History Clearance ($13.00), a Pennsylvania Criminal History Clearance ($22.00), and Federal Criminal History Clearance (fingerprint background check $26.20).

    Pennsylvania law, 24PS 1-111, specifies that ALL EMPLOYEES of public and private schools including those of independent contractors, but excluding employees who do not have direct contact with students, hired as of January 1, 1986 must undergo background checks. In order to avoid delays in processing the background checks, job applicants must carefully follow the procedures listed below. Administrators may employ applicants on a provisional basis for a single period not to exceed a period of ninety (90) days provided all conditions listed in the Law are met. All employees of schools, whether residents or non residents, must complete all three clearances. 

    FILING PROCEDURE: PA CRIMINAL HISTORY RECORD CHECK

    1. In order to file by mail, applicant may pick up Form SP4-164 from the any school district, PA State Police Barracks or from the PA State Police web site at psp.pa.gov
    2. Applicant completes Part I only. Include your name, address, and telephone number as the requester. Do not use the name or address of the Department of Education as the requester. Under the REQUESTER IDENTIFICATION section, check the box for Individual/Non-criminal Justice Agency. Under the REASON FOR REQUEST section, check EMPLOYMENT. Enclose a certified check or money order for $22.00 payable to the Commonwealth of Pennsylvania. NO CASH OR PERSONAL CHECKS WILL BE ACCEPTED. Mail the application with payment to the following address: Pennsylvania State Central Repository-164, 1800 Elmerton Avenue, Harrisburg, PA 17110-9758. Forms will be processed by State Police as they arrive. The State Police will return the Criminal Record Check to the applicant by mail within approximately four weeks.     3. Applicant presents background check information to prospective employer. The ORIGINAL should be RETAINED BY THE APPLICANT and may be used for all employment verification for the one year period after issuance. The school entity shall make and keep a copy of the original.
    4. This application may also be filed on the internet at the Pennsylvania Access to the Criminal History (PATCH) website https://epatch.pa.gov/home  Applicants using the website are required to use a major credit card. Payment of $22.00 may be made with Visa, Discover, Master Card or American Express. All customers must have a valid email address in order to receive vital information from the PATCH administrators. Please note down the Control Number and the Request Date; this information will be required in order to inquire on the status of a record check. Once a criminal history check has been requested, one of the following possible responses will be immediately received:

    Pending indicates that a response did not come back quick enough. If this is the response received, please check the status at a later time.

    No Record indicates that there is no criminal history information contained in the files of the Pennsylvania State Police Central Repository. The "Certification Form" for No Record should be printed and used as the Original.

    Request Under Review indicates that the user must periodically check back to determine the final status which would be either No Record or Record (mailed to the requester).

    FILING PROCEDURE: PA CHILD ABUSE HISTORY RECORD CHECK

    1. Applicant may pick up Form CY 113 from any school district or file the application here: https://www.compass.state.pa.us
    2. Type or print clearly and neatly in ink Section I only. All information must be completed in full. All previous names, addresses, and household members since 1975 must be provided to the best of your knowledge and belief. Application must be signed. Check the school block for Purpose for Clearance. Do not check more than one block. Enclose a certified check or money order for $13.00 payable to the Department of Public Welfare. NO CASH OR PERSONAL CHECKS WILL BE ACCEPTED. Mail the application to Childline and Abuse Registry, Department of Public Welfare, P.O. Box 8170, Harrisburg, PA 17105-8170.
    3. Applicant presents background check information to prospective employer. The ORIGINAL should be RETAINED BY THE APPLICANT and may be used for all employment verification for the one year period after issuance. The school entity shall make and keep a copy of the original.
    4. Clearance results will be mailed to you at the current address noted on your form within 14 days from the date that the clearance is received in the DPW office. For problems or questions call 1-717-783-6211.

     

    FILING PROCEDURE: FEDERAL CRIMINAL HISTORY RECORD CHECK (FBI FINGERPRINT)

    Applicants MUST register via the following: https://uenroll.identogo.com/ 24 hours/day, seven days per week / 1-844-321-2101 – 8am - 6pm EST.

    When registering you must enter the appropriate service code for PA Department of Education - 1KG6XN

    The applicant will pay a fee of $26.20 for the fingerprint clearance and receive a copy free of charge.

    Credit/debit card may be done online. Money orders or cashiers check payable to Morpho Trust will be accepted at the fingerprint centers. NO cash or personal checks allowed.

    • Applicants must provide proof of identity upon arrival at the Fingerprint Center such as a state issued drivers license, state I.D card, passport, etc.
    • Once registered, you may have your fingerprints taken at any of the locations listed on their website:  www.identogo.com/locations  OR  Lincoln Intermediate Unit #12

    • Applicants must provide Employer with their UEID (Universal Enrollment ID). FASD will then obtain the original clearance via the website.
